Email the completed form together with the original receipt as proof of purchase; to HYLA (M) Sdn Bhd at email@example.com within 7 days from the expiration of 30 days' risk-free date.

1. This offer is only open to individuals who have purchased an eligible HYLA EST Air & Room Cleaning System via official HYLA Malaysia Online Store at www.hyla.my and attended to a session of “User Training” of which conducted on the same day of delivery by our qualified authorized personnel, thereafter have used it for a maximum of thirty (30) days in accordance with the operating instructions. If after thirty (30) days of using the product and it does not perform exactly as described, you may apply for money back in accordance with these terms and conditions.
2. It's a compulsory to submit the 30 Days Risk-Free Money Back Guarantee Claim Form to process return. The Claim Form can be downloaded here. Should you encounter any problem relating to form submission, please contact our HYLA Careline at 1300-13-HYLA for assistance.
3. The "30 Days Risk-Free" period runs effectively from the date of delivery.
4. Original receipt(photocopies are not accepted), as proof of purchase must be produced and return to us upon inspection of the goods by our authorized personnel once your refund is approved.
5. Money back will be paid by cheque, for the price paid (excluded bank charges or interest if any), within a period of 30 days of receipt of the product and completed documentation.
6. No responsibility can be accepted for claims lost or delayed in the return or incomplete or illegible forms. Proof of posting will not be accepted as proof of delivery.
7. All care should be taken to ensure the intended returned product is adequately packaged in its original packaging to avoid loss or damage. We reserve the rights to deduct from the refund the cost of rectifying any loss or damage to the product.
